IntroductionWelcome to the June 2016 issue of the Clarity Assure eBulletin. It contains the latest news and advice relating to the Quality Improvement Service for North East. 2015/16 Year-endApril 2015 – March 2016 data is due to close on 26th July 2016. Please let us know if you are anticipating any problems with completing data entry for both CAP and Sepsis. CAP CURB Score ChangesWe would be very grateful for any feedback on the potential CAP changes which are currently being discussed: We need to determine whether having no CURB score recorded should mean an automatic fail for the antibiotics measure, or if the record should be able to pass the antibiotics measure without a CURB score if the patient was given antibiotics in line with local protocol. The proposed change would mean that the CURB questions are added to the antibiotics measure. One Trust proposed we also add in the new question (in bold): - Is there documentation within the clinical record that the patient received a CURB-65 OR CRB-65 assessment within 4 hours of arrival?
- What is the CURB-65 score documented within the patient's clinical record within 4 hours of arrival?
- What is the CRB-65 score documented within the patient's clinical record within 4 hours of arrival?
- If the CURB score was documented, was the antibiotic regimen in accordance with guidelines for the score recorded?
- Is there documentation within the clinical record that the patient received the appropriate antibiotic regimen according to current local guidelines?
If you could discuss this with your clinical teams it would be very helpful. If there are any other suggestions for additional questions we would be keen to hear these also. We would like to hear views from all Trusts if possible so that we can try to work out a regional agreement. Sepsis CQUIN Changes on AssureWe are also keen to hear your feedback on the development of changes for the new sepsis CQUIN which need to be implemented on Assure by the end of July. Again there has been ongoing discussion across the region but we are keen to hear from all involved Trusts. Please get in touch to ensure your Trust is represented. |
