Patient removals and amendments_NE

PLEASE NOTE:  Amendments can only be processed by Clarity when the request is received from an email address registered to an Assure user account.

Patients can be removed from Clarity Assure if the diagnosis coding has been changed so that they no longer apply to a measured focus area. 

To remove a patient please log into the Clarity Assure Helpdesk via the Clarity Assure system and submit a ticket with the......

  • Patient ID (not NHS number)
  • Focus Area to be removed from
  • Admission Date
  • Discharge Date


We have created a form to assist you in sending your removal requests to Clarity.  This form must be submitted together with the evidence as detailed in the above guidance via the Helpdesk within Assure.

PLEASE DO NOT SEND PATIENT IDENTIFIERS VIA THE EMAIL SYSTEM AS THIS IS NOT A SECURE METHOD, PLEASE USE THE HELPDESK WITHIN ASSURE INSTEAD.

ALSO PLEASE REMEMBER TO REMOVE ANY PATIENT IDENTIFIABLE INFORMATION SUCH AS NAMES, DATE OF BIRTH, GENDER, NHS NUMBER.
