LearningFromDeaths - QASpecific
This report uses the same method as the Learning From Deaths report but enables users to focus on (include) a particular dataset rather than exclude them.
User will allocate a relevant scoring question to determine if that question subject was avoidable. They can then opt to excluding selected questions and answers from the dataset as well as allocate a scoring question and answer combination to show good and poor care.
PLEASE NOTE:
- This report is best used with review forms including more than one scoring question.
- The report is only available to be ran at a Trust Level (not site level).
- Works for any review form where there are Select Lists and Form Scores.

Report Data Explanations All the figures as based on the month in which the death occurred for the review form selected and meeting inclusion question / answer selections | ||||||||
---|---|---|---|---|---|---|---|---|
Month of Death | Total Number of Reviews | Number of Priority Reviews | Number of Priority Reviews Completed | Total Number of Completed Reviews | Where Death Judged To Be Avoidable | Where Death Judged To be Not Avoidable | Reviews Where Quality of Care Was Graded As Good | Reviews Where Quality of Care Was Graded As Poor |
After applying the Inclusion criteria....... | ||||||||
The month in which the death occurred (e.g. 2017-06-01 = June 2017) | How many of the selected review forms have been assigned | How many of the Total number of reviews have a priority flag | How many of the reviews with a priority flag have a status of "Complete/Closed" Also expressed as what percentage this is of the total reviews | How many of the Total Number of Reviews have a status of "Complete/Closed" Also expressed as what percentage this is of the total reviews | How many reviews have a status of of "Complete/Closed" and have the answers selected in the "Select Answers Categorised as Avoidable" Also expressed as what percentage this is of total completed reviews | How many reviews have a status of of "Complete/Closed" and have the answers selected in "Select Answers Categorised as Not Avoidable" Also expressed as what percentage this is of total completed reviews | How many reviews have a status of of "Complete/Closed" and have the answers selected in "Select Answers Relating To Good Care" Also expressed as what percentage this is of total completed reviews | How many reviews have a status of of "Complete/Closed" and have the answers selected in the "Select Answers Relating To Less Than Good Care" Also expressed as what percentage this is of total completed reviews |
Example
(Using the same example as that in the original Learning from Deaths report so you can see the difference)
Scenario: I work for Trust 1 and want to run a report to see what level of care all cases with a definite or suspected learning disability received. I want to see the care given for a calendar year basis from month X with a Y review assigned, where the death was avoidable.
My Case Records have the following
Month of Death | Review Form | Priority Type | Review Status | Did the patient have a learning disability? | Scoring Question - AVOIDABLE DEATH? In your judgement, is there some evidence that the patient's death was avoidable if the problem/s in healthcare had not occurred? | Scoring Question- GOOD CARE? How would you rate the OVERALL quality of healthcare received from this trust? |
---|---|---|---|---|---|---|
month X | Y review | Standard | Complete | No learning disability | 6-Definitely preventable | 1-Excellent |
month X | Y review | Standard | In Progress | No learning disability | 1-Definitely not preventable | 6-Very poor |
month X | Y review | Priority | Complete | Clear indication of learning disabilities | 4-Probably preventable greater than 50-50 | 2 - Good |
month X | Y review | Priority | In Progress | No learning disability | 1-Definitely not preventable | 6-Very poor |
To see this I would enter the report criteria as follows:-
Report Field | Criteria Selected |
---|---|
Select Organisation | Trust 1 |
Select Review Form | Y review |
Report From | month X |
Standard or Fiscal Year | Standard |
Select Inclusion Questions | Did the patient have a learning disability? |
Select Inclusion Answer Options | Clear indication of learning disabilities Some indication of learning disabilities |
Select Question Relating To Avoidable/Non Avoidable Death | In your judgement, is there some evidence that the patient's death was avoidable if the problem/s in healthcare had not occurred? |
Select Answers Categorised as Avoidable | 4-Probably preventable greater than 50-50 5-Strong evidence for preventability 6-Definitely preventable |
Select Answers Categorised as Not Avoidable | 1-Definitely not preventable 2-Slight evidence for preventability 3-Possibly preventable less than 50-50 |
Select Question Relating to Grade of Care Provided | How would you rate the OVERALL quality of healthcare received from this trust? |
Select Answers Relating To Good Care | 1-Excellent 2-Very good 3-Good |
Select Answers Relating To Less Than Good Care | 4-Adequate 5-Poor 6-Very poor |
This means that the figures in my report will disregard the cases where there was no identified learning difficulties before producing any figures.
My report data would looks as follows (Grey cells are to remind you what the figures are for)
