Professional profile
The Professional profile can be used to document your professional skills and qualifications. The ‘Online CV’ feature allows you to collate and download all of the information you add, forming a structured CV listing your professional details, and any date relevant information will automatically be added to your appraisal.
Tip: Whenever you are awarded a new qualification or learn/practice an extended skill, you should enter the information into your Professional Profile. This means you will always have quick access to your certificate information and can monitor your expiry dates to ensure your skills are renewed at the correct time. If there are particular sections/lectures/training courses that you found particularly beneficial or interesting, you can document them more thoroughly in your Portfolio.
Your Professional Profile is separated into 5 sections, all of which must be completed for your appraisal.

Personal information
Edit your personal information and click 
. This information will be automatically added to your appraisals.
Qualifications
Use this section to add details of any professional qualifications you hold that are relevant to your appraisal. You can upload electronic or scanned certificates and documents once you have added a qualification.
Qualifications will be automatically added to your appraisal.

The form is separated into two tabs:
Add the details of your qualification. Only the Title and Date Awarded fields are required but you may find it useful to include a description for your Appraiser. Once you have completed the fields, click 
at the top of the page.
Extended skills
Use this section to record any extended skills you demonstrate. This information will be automatically added to your appraisal.

Add the details of your extended skill. The Name, Clinical Area and Qualification Date fields are required but you may also find it useful to include additional details for your Appraiser. Once you have completed the fields, click 
at the top of the page.
Memberships
Use this section to record any memberships you have to specialist associations or faculties.

Memberships will be automatically added to your appraisal.
Roles
Your appraisal should focus on all of the professional roles you perform, not just your clinical positions. Use this section to add information about all of the roles you perform, including managerial, academic, teaching and research positions.
You can then upload any documents as evidence for your appraisal.

You should complete as much of the Details tab as possible to provide background for your Appraiser. Once you have completed the fields, click

Probity
The Probity section involves details of issues relating to honesty, integrity and being trustworthy; characteristics which are integral to a nurse's practice. Probity issues may involve both the nurse and their practice. An effort should be made to reflect on at least one issue of probity each appraisal year. Matters of probity may include:
- Fabrication or falsification of documents such as; information regarding medical services, CVs, research, report writing
- Ethical practice including bribes regarding drug representatives and other practitioners
- Patient care such as; exploiting vulnerable patients, receiving gifts, confidentiality issues
- Conflicts of interest such as financial gain from organisations they are affiliated with

It is important to declare all probity issues you encounter through your practice and to reflect upon them for your appraisal. Any information added here will be added to the Probity declaration in the Statements and Declarations of your appraisal.
Online CV
Click 'Online CV' in your left navigation bar to download a CV containing the information from your Professional Profile. To see an example PDF, click here.
