Portfolio

Your Portfolio is your personal learning log. You can record information in your Portfolio to record activities and decide at a later date whether to submit this information as part of your appraisal; information in your Portfolio is not visible to your Appraiser or Administrators.

Tip: You should aim to update your Portfolio regularly. You will benefit most if you enter the details of an event/activity/feedback soon after it occurs as it will be fresh in your mind. You can use the Reflection tab to give a structured reflection on the event and help process your thoughts and feelings on the topic, which will help you address any issues or possibilities that you may have otherwise missed.

The Portfolio can be particularly beneficial after completing a quality improvement activity. The structured templates in the Details tab can assist with a post-activity write-up to help make sure you do not miss details of an important entry, documents can be uploaded and then the Reflection tab guides you through giving a thorough reflection.  The Six Cs tab will enable you to keep track of which competencies you are excelling in, and which you may need to demonstrate more, which will, in turn, benefit your day-to-day practice.

What does it contain?Your supporting information from all of the appraisals you have completed through Appraisal Toolkit for Nurses and any other items you have added that have not been linked to an appraisal.

Only date-relevant evidence, including your:

  • personal details
  • information about your roles and responsibilities
  • supporting information that you have added directly to your appraisal or imported from your Portfolio and additional evidence.

After your Appraisal meeting your Appraiser will add your Appraisal Summaries (Form 4)

Can I reflect on my items?NoYes – You can reflect on individual items or on a whole section.

Click "Portfolio" in your top menu bar to be taken to your Portfolio Overview page.

 


The overview page which shows every item you have added to your Portfolio and any items you have added directly to your appraisals.


You can find items in your Portfolio using the search function to search according to title, or you can click  to search by date or linked appraisal.
