Data Dump - Review Scores

Shows the answers to scoring/grading questions with each scoring value given it's own row.

Report Criteria Options
Organisation Overall	Select whether to run your report at top level YES will show the data within the report at the top level (e.g. Trust Level) NO will show the data within the report at a lower level (e.g. site level)
Organisations	Depending on the option selected in Organisation Overall you can then select the relevant Trusts/Sites
Review Form	This will show all review forms within the system available to the Trusts/Sites
Period From	This will show a list of all batch options (by month of review start date) within the system for the relevant Trust/Site and Review Form
Period To	This will show a list of all batch options (by month of review start date) within the system for the relevant Trust/Site and Review Form
Report period type	Select to see data for reviews with priority statuses of All Standard Priority

Report Data Explanations
OrganisationCode	Site code the data is assigned to
FormName	The Name of the review form the row data relates to
Discharge Month	The discharge month case record the review form data relates to
Case Review Identifer	The CaseID of the case record the review form data relates to
Case Review	This is an anonymised ID for cross referencing purposes and to show it is data for the same case record as another data row
Review	This is an anonymised ID for cross referencing purposes and to show it is data for the same case record as another data row
Priority Review	This shows whether the selected review form was flagged a a priority review
Case Review Status	The status the review form was at when the report was ran
Review Start Date	The first initial that the review form was saved
Review Score	The scoring title of the grading question
Recorded Outcome	The answer provided to the scoring question
