Appraisal evidence and supporting information (Forms 1 - 3)
Form 1 is split into 4 sections. It contains the appraisee’s:

None of these sections require review – they are provided for information purposes only.

- Roles
- Minor surgeries/surgical logs
- Last Year’s PDP
Please note: Before you can sign off the appraisal, each of the appraisee's Last Year's PDP items must have a status (e.g. complete/proposed carried forward/proposed dropped). Click here for instructions.
Form 3 contains the appraisee’s appraisal evidence, including their MSF and complaints and compliments.
- Use the Appraiser Notes function to annotate items in preparation for your meeting.

Form 4 - Appraisal summaries
As the Appraiser you must complete the three sections in Form 4 after the appraisal meeting. For more information about Form 4, click here.
