This report is designed to enable users to produce a report using the scoring questions.

User will allocate a relevant scoring question to determine if that question subject was avoidable.  They can then opt to excluding selected questions and answers from the dataset as well as allocate a scoring question and answer combination to show good and poor care.

PLEASE NOTE:

  • This report is best used with review forms including more than one scoring question.
  • The report is only available to be ran at a Trust Level (not site level).
  • Works for any review form where there are Select Lists and Form Scores.

Report Criteria Options

(Coloured cells show the grouped/related questions)

Select Organisation

Select the relevant Trust code

Select Review FormSelect the relevant review form for your report data
Report FromSelect the starting batch month you wish to report on
Standard or Fiscal Year

Select the time period you wish to run your report on.

Standard = January to December

Fiscal = April to March

(Based on your selected report from date will also group the data in to standard or fiscal quarters)

Select Exclusion Question

This will show the list of all single select list questions (where you can only select one answer from a list of options).

Select the question you wish to exclude from the report data.

If you do not wish to exclude any questions select "No Selection"

Select Exclusion Answer Options

This is linked to your selection in the "Select Exclusion Question" field

It will show all of the answer options for the selected exclusion question.

Select all the answers which confirms they met the exclusion. 

If you do not wish to exclude any questions select "No Selection"

Select Question Relating To Avoidable/Non Avoidable Death

This will show the list of all scoring questions within the review form(s) selected.

Select which scoring question you wish to allocate as concluding whether a subject area was avoidable or not avoidable

Select Answers Categorised as Avoidable

This is linked to your selection in the "Select Question Relating To Avoidable/Non Avoidable Death" field

It will show all of the answer options for the selected exclusion question.

Select all the answers which you want to deem as avoidable

Select Answers Categorised as Not Avoidable

This is linked to your selections in "Select Answers Categorised as Avoidable" field.

It will show the remaining list of answers which were not selected in the "Select Answers Categorised as Avoidable" and will be automatically selected for you.

Select Question Relating to Grade of Care Provided

This will show the list of all scoring questions within the review form(s) selected.

Select which scoring question you wish to allocate as indicated whether care given was good or poor

Select Answers Relating To Good Care

This is linked to your selection in the"Select Question Relating to Grade of Care Provided"

Select which scoring question you wish to allocate as concluding good care was given

Select Answers Relating To Less Than Good Care

This is linked to your selections in "Select Answers Relating To Good Care" field.

It will show the remaining list of answers which were not selected in the "Select Answers Categorised as Avoidable" and will be automatically selected for you.

All the figures as based on the month in which the death occurred for the review form selected and organisation selected minus any reviews meeting exclusion question / answer selections

Example

Scenario: I work for Trust 1 and want to run a report to see what level of care my cases received for a calendar year from month X with a Y review assigned, where the death was avoidable.  However, I do not wish to include any cases where there was a definite learning disability


My Case Records have the following
