This report uses the same method as the Learning From Deaths report but enables users to focus on (include) a particular dataset rather than exclude them.

Report Criteria Options

(Coloured cells show the grouped/related questions)

Select Organisation

Select the relevant Trust code

Select Review FormSelect the relevant review form for your report data
Report FromSelect the starting batch month you wish to report on
Standard or Fiscal Year

Select the time period you wish to run your report on.

Standard = January to December

Fiscal = April to March

(Based on your selected report from date will also group the data in to standard or fiscal quarters)

Select Inclusion Questions

This will show the list of all single select list questions (where you can only select one answer from a list of options).

Select the question(s) which represents the dataset you wish to focus the report on.

If you do not wish to exclude any questions select "Select All"

Select Inclusion Answer Options

This is linked to your selection in the "Select Inclusion Questions" field

It will show all of the answer options for the selected inclusion questions.

Select all the answers which best suits your inclusion requirements

If you do not wish to exclude any questions select "Select All"

Select Question Relating To Avoidable/Non Avoidable Death

This will show the list of all scoring questions within the review form(s) selected.

Select which scoring question you wish to allocate as concluding whether a subject area was avoidable or not avoidable

Select Answers Categorised as Avoidable

This is linked to your selection in the "Select Question Relating To Avoidable/Non Avoidable Death" field

It will show all of the answer options for the selected exclusion question.

Select all the answers which you want to deem as avoidable

Select Answers Categorised as Not Avoidable

This is linked to your selections in "Select Answers Categorised as Avoidable" field.

It will show the remaining list of answers which were not selected in the "Select Answers Categorised as Avoidable" and will be automatically selected for you.

Select Question Relating to Grade of Care Provided

This will show the list of all scoring questions within the review form(s) selected.

Select which scoring question you wish to allocate as indicated whether care given was good or poor

Select Answers Relating To Good Care

This is linked to your selection in the"Select Question Relating to Grade of Care Provided"

Select which scoring question you wish to allocate as concluding good care was given

Select Answers Relating To Less Than Good Care

This is linked to your selections in "Select Answers Relating To Good Care" field.

It will show the remaining list of answers which were not selected in the "Select Answers Categorised as Avoidable" and will be automatically selected for you.
