The following process shows how to add items (if you are in your Portfolio or in your Appraisal).

Please note: if you add an item to your Portfolio you must import it into your appraisal if you want your Appraiser to see it

To add an item:

  • Click the item type in your left navigation bar
  • Click  at the top of the table (in Quality Improvement, click  then select type from the drop down list)
  • Complete the Details tab
  • Upload Click any supporting documents into the Documents tab and click

Your Organisation may have enabled the GMP tab. This allows you to map your item against aspects of GMP. Simply tick the boxes next to the aspect of GMP you wish to link this item to and click 


If you are adding the item to your appraisal you can also:

  • "Claim Impact” in the Details tab if the activity has impacted your practice as a clinician. This doubles the CPD credit score for the item. If you claim impact you must explain why in the “Description” text box. (secondary care organisations with the appropriate settings)

  • Mark your CPD points as internal or external (secondary care organisations with the appropriate settings only)



  • Complete the Reflection tab

  • Click on the evidence type in your left navigation bar
  • If you have items in your Portfolio that fall within this appraisal period and can be imported, you will see a blue “Import” button. The number shown in brackets is the number of items you can import.
  • Tick the boxes to the right of any items you want to import then click
  • The item(s) will appear in your appraisal instantly so you can view/edit the items and complete your reflections.

The import button will not appear if there is no available evidence to import.

  • Click  in the drop down menu to the right of the item
  • Click at the top of the page to return to the overview page.

If you are viewing the item in your appraisal the option will be to Remove the item. Clicking this will remove the item from your appraisal but it will still be available in your Portfolio to import at a later date.
